Hello, Peter Can a keto diet increase my LDL and total cholesterol level? It s not always a keto diet it’s mostly low in carbs and higher in fats and protein. I know food shouldn’t normally increase my cholesterol but it’s very strange because I ve never had problems until I’ve started this low carbs lifestyle. Wegovy for weight loss

Weygovy is a new weightloss drug. What is your opinion of the results they are promoting? I know it’s previously been used in a lower dosage for treating type 2 diabetes (under the brand name Ozempic). I do have metabolic syndrome (waist circumference, elevated cholesterol), am obese and on the cusp of pre-diabetes (glucose and A1C just a hair below the cutoff). I am definitely interested in trying it, and a like the idea it could potentially help with weight loss and potentially progression to pre-diabetes/diabetes but I want to get your take on it. My understanding is average weight loss is in the neighborhood of 15%, but Weygovy isn’t covered by insurance for weight loss and it is expensive. Worth giving it a try?

Lp(a) / progressed ASCVD management

Firstly, thanks for the recent AMA #34, What Causes Heart Disease. It’s a great public service - and also helps further illuminate my own situation. In brief: I’m 41, have very high Lp(a), family history of ASCVD, and ~70% blockage in right coronary artery per CT angiogram. That said, I have no symptoms - I’m active and otherwise in good health. I manage my lipids through lifestyle, but also through statins, ezetimibe, and PCSK9 inhibitor (Praluent). I also supplement with niacin, vitamin D, vitamin K2, and other supplements. …yet, ASCVD progresses due to high Lp(A), which has come down somewhat with the PCSK9 inhibitor but remains very elevated. At what point should someone in this sort of situation, consider preventative stenting or other forms of more aggressive management? What are the range of options and how do you think about the pros/cons? Also - would be great to get Lp(a) doc Sam Tsimikas for an episode one day. Thanks much!
