Relevance of a study with 100x dosage on real world effects

Hi Peter. Your recent article - https://peterattiamd.com/nr-supplements-and-cancer/ - discusses a recent study trying to tie NAD supplementation to cancer growth. In the results, a 100x dosage of the normal dosage produced a non-statistically significant and small ( 1.27 hazard ratio ) result. I am struggling to relate this to reality. The study did prove that NAD+ can be affected by oral ingestion, which seems to be confirming that these supplements are not just sugar pills. I haven't heard anyone suggesting we should ingest 30 grams of NR supplements daily. We do many things proven to increase longevity; if we did them 100x, we would die very quickly. How is this different? Calling NR supplementation into question based on this kind of study doesn't make sense to me. There has been a lot of negative press lately because of the changes in the landscape. David Sinclair has ties to a company that seems to try to monopolize the NR supplementation industry for profit - for example. I understand some of that controversy but not how it relates to the basic premise that NR supplementation is good for longevity. Normally you call out bias in all of your thinking, and this seems to be slightly biased at first glance. I admit that I'm also biased - in the other direction. I would love to hear a discussion of this, including all the latest results from David Sinclair, while being as objective as possible as always.

Negative health effects of circadian misalignment?

Hi again, another question about sleep, what is it specifically about circadian misalignment that is detrimental to sleep/health? E.g. if you get sufficient deep and REM sleep, is it still detrimental? An example would be that I often deviate from my circadian rhythm on exercise days due to time-shortage and go to bed considerably later than usual but these are also the nights that I tend to get the most deep sleep and good REM sleep, which is likely due to the exercise. Could it be that the exercise offsets the detrimental effects of circadian misalignment?

HI Peter! I love your podcasts - Thank you so much!I wish you could be my doctor! Anyway, I am in a bit of a quandary at this point. I am 69 yo woman with APOE3/4, C677T ++, BMI 28, A1C 5.9, Fasting insulin 8 (down this year from 12), LDL-C 180, HDL 76, homocysteine was 13 umol/l last year but now at 5 with methylation support supplements). So…..I have a high Lp(a) 201 nmol/l (down from 237 last year for no known reason), LDL-P 1735, (FH-F of aortic stenosis/arteriosclerosis/T2D) and I have a calcium-CT score of ZERO. I have resisted statins because of Lp(a), pre diabetes, APOE4 (FH-F dementia) and current CT score, but since your very recent post I am unsure if I should now seek to add them. Of course none of my doctors have any idea nor any interest in what kind of intervention (if any) might be appropriate for my case. Mostly out of curiosity I have recently added berberine 500mg BID as I wait for some kind of guidance and I have asked for APOB ( I have a gene variant for high APOB) plus Ox-LDL to be added to my tests. Also I have NO idea at this point what is the appropriate diet for my case since low carb seems to raise LDL-C and ferritin- ( Since menopause I have developed mild hemochromatosis H63D++), and low fat tends to raise blood sugar, insulin and Lp(a). (I also have gene variant ++ for NAFLD and have early radiographic results w/normal liver metrics.) My Dad made it to 88 but had developed a LOT of bad facts. My Mom is 93 in perfect health and her mother at 102, died of boredom. Sandwiched between these genetic marvels I am attempting to skirt the mines.

Max HRV

I know a higher HRV is what we are trying to get overnight, but is there a risk to a big spike in HRV? After I had Covid my Max HRV was 150-180 and averaging over 100. My typical Average is 70-80.

Salt

Can we have a salt episode? When and how to use LMNT, considerations for people with high blood pressure-is the water content enough to dilute the risk?, Is taking LMNT okay with gout if you are doing it away from carbs? Just considering past episodes and all we have learned about salt.
