Finding a ’medicine 3.0’ focused primary physician

How can I best find a doctor who is focused on health & longevity, proactive vs reactive medicine and who will look at my current state and test results vs where I want to be in 20 years rather than comparing me to every other 60 years+ patient.

Fat Adapted Athlete - Ironman Performance

Hello Dr. Attia, I saw a person ask for a deeper dive on Fat Adaptation and I'd like to add on to the request that perhaps you could cover this topic in more depth. I am coming at the topic from a little different angle, in that I'd like to confirm / deny / understand - my N=1 study of being fat adapted and competing in an iron distance triathlon. I have completed 2 full and 1 half IM without consuming anything on the course - except water and electrolytes. My last race I had my best individual performance. I believe I understand what is going on metabolically, but I'd like to know if what I've done is supported by the science in terms of being a healthy approach and perhaps a better understanding of what actually happened and how this fits into a strategy for longevity. It can't be a fluke that I completed a 15 hour event without any nutrition - especially carbs - and did not bonk, but in fact had no issues and thrived. I've heard some of your podcasts discuss Zone 2 - along with researchers Phil Mafetone (which I follow), Phiney, Volek - but I haven't come across this topic discussed specifically. If you have covered it, if the question screen could point me to the episode number that would be great. Lastly, I'm interested in understanding the impact of endurance training, like long-course triathlon and its effect on inflammation and cardiovascular disease risk. I have consistently high cholesterol measurements and I'm spending a lot of time trying to reduce inflammation risk to lower my risk of a heart disease event and am wondering how training for IMs fits into that equation. How do I learn and practice the science of longevity as a physician in training?

Hi Peter I've been following your work for a long time now and it has always been fascinating. I finished medical school in 2020 and I'm currently training in internal medicine. I would like to learn more about longevity so that I can incorporate it into practice when I finish training. I would like your advice on how I could become more knowledgeable in this aspect of medicine.

Niacin and Area Under the Curve

Hello team, I have been catching up on some of the ASCVD podcasts and have been pondering Niacin, especially in relation to Peter's Area Under the Curve concept, mostly because it's cheap and readily available in contrast to some other lipid lowering modalities. I realize what I am asking involves a degree of speculation as trials have not tried to answer this specific question. That said, if Niacin really does have a LDL lowering effect, do you think there would be any benefit to putting a younger person (think 30s or so and otherwise healthy) on Niacin in interest of achieving a small/moderate LDL/ApoB lowering effect that would add up to a cumulative risk reduction over the course of many years?
