Anabolic steroids for geriatrics with sarcopenia

Peter, could you please give us your thoughts on why we don't treat frail elderly with anabolic steroids? If in the last decade or less of their life anyway I wouldn't expect the side effects / complications to be much of an issue relative to quality of life and increased function, especially in elderly men.

Sufficient protein intake on two meals a day

Hi, Peter. Insert obligatory fawning and thank-yous for all you do, etc, etc. I am currently eating twice a day and love the weight control and mental focus that I find goes along with skipping breakfast, but I'm curious if I am shorting myself on protein intake. As background, I do body weight and dumbbell exercises six days a week, run, paddleboard, and swim. Nothing too intense, but definitely regular. Is there truth to the notion that there is a maximum amount of protein our bodies can absorb for muscle protein synthesis at one sitting? If so, what is this amount and is it static across populations, as we age, and as our activity type and levels change? And can you talk about how much time needs to pass between meals to "reset" the threshold, assuming there is one? Got any other advice for those of use eating twice a day as far as protein absorption goes?

Pseudo dementia

Hello, Just wondering if there could be a podcast dedicated to pseudo-dementia, especially in geriatric population. Experiencing it at first hand with my 88 years old mom, who was a very high functioning medical doctor, survived so many stressful situations in her youth (single parent household, mother imprisoned during Nazi occupation, witnessing bombing of Prague, communist regime) and social isolation during pandemic pushed her over the edge in her mental health, while physical health stayed intact. Lack of mental health providers in her home country makes the situation even more difficult. Wonder about worldwide occurrence of pseudo dementia during last 18 months. Thank you

Carbs/SHBG connection

If you are cutting carbs (or even calories), how often should you check your SHBG and free testosterone levels? Is there a glucose cycling protocol to follow to reduce the level of SHBG if you find it high?

Fasting is more complex

This is not a good question, but a responding comment would be appreciated. I am not convinced about a strong correlation between between the fed state and muscle mass retention (and the corollary fasted state). I fast almost 130 hours per week, but still have sufficient time to bring in an average intake 200g of protein per day. There is an open question of how much of those amino acids are not used as protein, so yeah, I may not be getting effective use of those 200g of protein. However, any loss of muscle mass that I may have had is probably much closer correlated to lower levels of training than lack of protein. In this regard fasting should not be considered a substitute for fitness training, tho, the converse may be more true. Fasting forces efficiencies. Do these added efficiencies add value? That question could be asked about any dietary restriction
