Autoimmune Deep Dive

Generally, I'm deeply curious about autoimmune diseases and what we know about how they relate, if at all, to metabolic dysfunction. In particular, I'd love to see a deep dive on the thyroid and hashimoto's (and how all that can relate to insulin resistance).

Role of Insulin in the menstrual cycle and PCOS

Hi! Have been learning about PCOS in medical school and there was a very brief mention of the role that insulin plays in PCOS and the menstrual cycle. They mentioned that in women with metabolic syndrome putting them on metformin actually increases their chance of ovulating. It was also mentioned that even a 5lb weight loss in patients with PCOS can help stabilize the menstrual cycle. I've seen some other questions in the forum related to insulin resistance, metabolic syndrome, PCOS, pregnancy and menstruation. Would love an AMA or interview with another expert in this field to explore the role that insulin plays in these topics. At what point would you put a non-diabetic woman struggling with either amenorrhea or infertility on metformin? Is the mechanism here solely related to metformin's insulin regulation or does the metformin have alternative mechanisms by which it might be affecting ovulation?

Diabetes and low level conic inflammation...

Not sure if you already have a podcast...I have been doing majority of things that Dr. Attia mentioned... Zone 2, strength training, 16 hr intermittent fasting, one day water only fast, and eating health. Lost almost 20 lbs in past 5 months. My doctor (functional and listens to Dr. Attia also) gave me a CGM trail via Dexcom. What I am finding is that even will all the changes I made my baseline glucose is about 115. That as I understand could be a function of low level chronic inflammation. I have been able to reduce my LDL significantly via 5mg of Rosuvastatin but that does nothing for my inflammation and/or sugar levels. Would be great if we can do a podcast on potential causes of low level inflammation and its connection to heart disease. From what I read and myself it appears there is one. Not sure if Dr. Attia is an knowledgeable in this area but would be great to have endourologist, GI and Dr, OKeef or Dr. Sinha talk about subject, especially around gluten and lectin.

Toxins, Pesticides, Ecotoxicology, impact on human health--recommending a professor.

Perchlorate, Manganese, PCBs, Mercury etc. One area that I think does not seem to be covered is environmental impact on our health. I think you should bring on Frank von Hippel PhD who runs a lab at Univ of Arizona and conducts research on ecotoxicology, mechanisms of toxicity, and health disparities (lots of research on endocrine system).
