If fasting boosts metabolic efficiency by increasing mitochondrial efficiency: how ? 

Why does it not last?  tealogically, fasting should force the body to make the effort to find higher efficiency configuration but it generally makes no sense to reverse those gains once nutrients are restored.  This seems to imply there is some kind of long-term higher cost to being more efficient when the body does not need to be.  What is really going on here? we know having unneeded muscle is very costly so makes sense for body to convert that excess to fat/energy, but why do the same thing for  mitochondrial efficiency as well?  Is it using a more valuable resource to stay efficient?

Blood Glucose - Next Steps

I enjoyed the recent AMA on all things blood glucose. What I would love to hear is a follow up. For those watching their numbers as suggested by Peter, what are the types of exercise or dietary changes that can decrease glucose variability, decrease the peaks, and lower the average?

Zone 2 Training

Dr Attia, first off, thank you!! I subscribe to zero podcasts except yours. I value several but there are simply too many Dave Asprey’s out there groveling and selling their soul for their benefit. I truly believe in your approach and follow your advice closely. I do have many questions however the most pressing is zone 2 training. I’ve listened to your podcast that discussed calculating my zone 2 rate but I am trying to dial it in. I am not in a place to get a lactate meter but would like a closer description of the HR to achieve. There are 2 ways to calculate HR training zones the 220-age x .8. There’s another that incorporates RHR in the equation. Can you please help clarify. I’m a Pelotoner and have incorporated my Peloton moniker “KickAss100YrOld” in honor of your influence on my goals of healthspan, lifespan, brain span, and longevity. I’m 57 with a RHR of 61. It used to be 58 but I developed an ITBand issue attempting to maintain a 144 HR for an hour 4 days a week. In order to get that HR I had to stand up for periods of 35-45 minutes at a time and low and behold 2 months ago 2 months into the standing up the ITBand inflamed and I’m just now getting back to normal. I’m very interested in your zone 2 qualifier and completely understand that zone 2 on the Peloton is not the zone 2 you discussed. Please help if you you can. Thank you for all you do. My life has improved tremendously even to the point my PCP cannot provide the Ion panel that I am getting from a functional med MD because you suggested to begin at minimum with more than your regular blood panel, which by the way is dialed in thanks to all of your advice. Help if you can.
