Immune systems - why such differences in how often people get sick

Why do some people get sick much more easily and is there anything they can do to strengthen their immune systems? Here I mean normal people, not with severe medical issues. My wife catches every bug going and is laid up with them, I dont catch any of them.

Energy Model of Calorie Intake but Specific CGM Goals?

I've submitted something similar before, but I hear Peter again in this current AMA suggest the energy model means that after protein is covered, carb/fat mix does not really matter. That just seems a bit incomplete or misleading given the importance he places upon glucose management and insulin resistance, learnings from CGMs etc. He has stated goals of minimizing post-prandial spikes and lowering fasted and average glucose levels then it seems from my own CGM experience (and everyone else I've talked to) that the amount of carbs in one's diet definitely matters (along with quality of those carbs, fiber and sugar content, potentially order in which you eat, probably other factors...). Can he please address or clarify this issue at some point? Thanks!

EAAs or BCAAs during fasting?

I know Peter has chjanged his views on BCAAs over the years, recently telling Tim Ferriss he no longer takes them. But if he's recommending small maintenance dose of whey protein during fasting (as mentioned on this week's AMA), why not amp that up with some BCAAs/EAAs as well, which have even fewer calories? In any event, probably worth updating his listeners on his views here. Thanks!

Male Life Expectancy

Despite calls for increased funding of women's health initiatives, on average women live almost 5 years longer than men. While behavioural factors (ex: drinking, suicide) may not favour men, this difference is seen across many mammal species. Are there any insights into why women have longer life expectancies than men? Is there any research underway that may provide some insight? In some ways, couldn't women be the perfect control group to study male life extension?

My Afib diagnosis

I was diagnosed with afib back in 2017 during a routine annual physical. If it hadn’t been because of the ekg, I would have never known. 8 year later, I remain asymptomatic. I’m an avid cyclist and log about 200 miles per week and do at least one or two HIIT workouts on the trainer during the week. Two of my group rides reach efforts well into Z’s 3-5, and one of them is usually no less than 55 miles. My total cholesterol is 187 and everything else is normal. I just turned 65. What advise can you give me?

Hi Just listed to this. As a long-term sufferer of tendonitis of the elbow/forearm it was interesting to hear about the use of radiology for such conditions. I was, however, disappointed to listen through the prostate cancer portion. Yet again it seems like the state-of-the art in the US is way behind what's being done in Europe (in the field of radiology and other techniques) to treat prostate cancer. I'd really encourage Peter to try to have someone like Professor Dr Carlo Greco of the Champalimaud Centre in Lisbon on a podcast to discuss his use of Image Guided Radiation Therapy (mentioned in passing in the podcast) to treat prostate cancer. This is (most often) single treatment, targeted radiation WITHOUT androgen deprivation therapy. Carlo is a wonderful guy. https://fchampalimaud.org/clinical_services/radiotherapy Such treatment was one of the options I considered to deal with my own prostate cancer (Gleason 3-4). Ultimately I decided to have High Intensity Focused Ultrasound with Professor Mark Emberton in London. However, a close friend who just recently was diagnosed with prostate cancer (Gleason 4-5) is being treated by Carlo later this month. He will be the second person I know treated by Carlo and his team. His actual treatment will take about 15 minutes with a period of prep the day before. One and done, no androgen deprivation therapy. Yes, these are focal therapies and carry a risk of recurrence but the lifestyle gains are substantial (no ED issues, no incontinence etc). Carlo et al can discuss their experience with respect to patients needing follow up treatment. (In my own case, 1/3 of my prostate was ablated using HiFu in June '23. My last MRI in January this year showed me cancer free.)* Regards * MRIs in the US are another kettle of fish. Twice now I've had to have prostate MRIs, initially done in the US (two different institutions), repeated in London because the imaging team in London considered the quality of the scans from the US very poor. I'm not even bothering with additional scans here in Miami anymore. Often we pay a lot for less in the US.

Absolute risk versus relative risk

It’s interesting how Peter belittles the statistics about breast cancer and HRT in the Women’s Health Initiative, yet the Jupiter trial looking at statin therapy for LDL has similar results regarding MACEs. Shouldn’t we try to be more consistent in how we interpret data? Personally, I think HRT should be offered to almost every woman, and I don’t think everyone needs a statin. I’m concerned about its effect on how muscles handle glucose, and how many people seem to perceive cognitive impairment on them. Statins haven’t had a dramatic impact on the incidence of cardiovascular disease, but we’re sure having an epidemic of diabetes and dementia.

Slow-motion lifting and MEDEX machines

You have mentioned you do slow-mo bench press with dumbbells. Can you double click on the benefits of slow-mo lifting, which exercises are good/bad for slow-mo, whether it's good for strength, hypertrophy or muscle endurance? Also, have you trie MEDEX machines, that give you feedback as you lift? Thanks!

I've just completed 10 days of CGM. My CGM% is 5.9. What does that mean and how do I compare that to AC1. As well, what is a tolerable differential (spikes) and what is an acceptable number of spikes in a day?

Creitine effect on HPT and urinary issues

Can taking creating lessen Medium HPT suptoms ? Have studies been done that showed any conclusions ? When I took my first dose of creating my urgency to constantly have to urinate almost completely disappeared. The best dat I’ve had in several years. My physician doesn’t really know much about the world of exercise or the supplements. He encouraged me to proceed with the TURP in mid July. Thinking of rescheduling, but I probably can’t afford take creating my whole life{I’m 75} but I will have to pee the rest of my life. On the fence about the surgery, but I’m really gonna do it. Carotid sonogram or MRA carotids in asymptomatic person

I'm an 80 yo retired radiologist with 3 vessel coronary artery disease and CTA or coronaries over 2000 with no coronary symptoms. Coronary angio showed no lesions of 50% or greater. I have well controlled hypertension and well controlled hypercholesterolemia with rosuvastatin, ezetimibe, and repatha. As a side note, I have always exercised alot and still walk 1.5 miles a day, ride the bike 10 miles a day and do some of the p90x workouts weekly but usually only the first 30 mins. I only became a real advocate for my own health with my internist after beginning to listen to your podcasts, not previously realizing it was not okay to be within normal limits. After hearing your podcast about half the people who die of atherosclerotic coronary artery disease have death as their first recognized symptom. I then asked my internist to order a CTA or the coronaries for which he referred me to a cardiologist. After the CTA was non-diagnostic due to the high calcification I requested a coronary arteriogram. Though my cholesterol and triglycerides were wnl range I pushed to add ezetimibe and repatha. Diet / Exercise Protocol for Preventing Recurrence of Glioblastoma

Hey Peter! My dad had another clean scan this week after surgery and treatment for glioblastoma. I know there's no cure but what diet / exercise protocol would you recommend for someone looking to delay recurrence as long as possible. He was on rapamyacin before treatment and isn't sure if he should go back on. Thanks in advance!

ANA Pattern - Function Health Biomarkers

I recently got my results back from my blood test with Function Health and my ANA Pattern was out of range (mitotic, intercellular bridge). I reviewed AMA #59 on inflammation and you did not mention this test/biomarker as something you typically review. Has that changed since that episode was released? Is this a test that you occasionally run? Would love your thoughts on why you do or don’t run it and if it’s something that can be beneficial.

Recent developments in blood screening.

My daughter, a PCP in Los Angeles , recommended that my wife and I take a cancer screening test by Callari. She works in a concierge practice and employ the test with all of their patients. My PCP in Pa is hesitant to prescribe it because of his concern for false positives. This test among others was featured in a recent WSJ article on their rapid growth . I think a deep dive with experts would be a great subject for an Ask Me Anything poscast. Thanks.

Have I missed coverage of Alzheimer’s risk factors

As someone who’s followed your work for 15 years, I appreciate your precision in distinguishing correlation from causation. In the Alzheimer’s risk landscape, APOE4 is clearly dominant but immutable—while fasting insulin shows up as both highly significant and modifiable. Yet it seems relatively underrepresented in cognitive health discussions, including yours. Is this due to gaps in the research base, unresolved causality, limitations in clinical translation—or gaps in my uptake of your coverage?

Cancer screening

Recently you recommended annual lung CA screening (CT scan). Which would be better / recommended a whole body MRI or CT lung given cost is not a limiting factor.
